Your ad rank, which is quality score multiplied by max bid, will mainly determine the position of the ad. The ad with highest ad rank is placed at first position.
When you divide the next biggest ad rank that is below you using the quality score, you get real CPC. However, in case you happen to be the only one bidding or you have the lowest amount of bid in a particular auction, then that rule does not apply. In that case, you need to pay your highest bid for clicks. In case you have low quality score yet decide to bid, you may be highly penalized.
